HARRISBURG, Pa., Oct. 3, 2013 /PRNewswire/ -- The Geneia Institute today announced a partnership with the Hamilton Health Center to help it succeed as a patient-centered medical home. Rooted in the goals of improving the quality and cost of health care for Hamilton's patients, the partnership includes comprehensive education and training for the Center's administrative and clinical staff.
The Hamilton Health Center, a Federally Qualified Health Center that serves the residents of the Greater Harrisburg area with medical, dental and related social services, created a medical home to provide a higher level of coordinated care to its patients. Medical home patients work closely with a chosen primary care physician, a medical assistant, and a care team to manage and improve their health.
"Our health care system is in the midst of major transformation, one that requires a strong and simultaneous focus on improving patient health and satisfaction and costs," said Heather Lavoie, President of Geneia. "At The Geneia Institute, we know that implementing this level of change is challenging work, and that's why we're proud to partner with organizations like the Hamilton Health Center which are at forefront of this evolution."
During the partnership, The Geneia Institute will educate the Hamilton Health Center staff on the medical home model of care delivery and will provide skill-based training to support the medical assistants and their managers in their expanded roles as members of a care team.
"At the Hamilton Health Center, we have embraced the patient-centered medical home because we believe that the model, if implemented effectively, will help us to better serve our patients. The Geneia Institute's work to prepare and train our staff ensure that we are well-positioned to succeed," said Jeannine Peterson, CEO of Hamilton Health Center.
